Registration
Download the 2009-2010 Registration Form. You will also need to download the Class Schedule (see below) in order to complete your Registration Form. Please read all Agreements of the Registration Form carefully before returning your application to Dance Arts. In order for your registration to be accepted, the Registration Form must be signed and the application must include any registration fee and payments as instructed in the application.
After completing the Registration Form, you may mail or hand-deliver it to:
